To request an appointment fill out this form. We will be in touch to confirm your date and time. Appointments are not finalized until you have received a confirmation email with your date and time.

If your requested date and time is unavailable, we will reach out to suggest alternative times.

Please be sure to include the full address of the location where you'd like to receive your MFR treatment. This will help us determine availability.

We currently do not charge a travel fee for clients within a 12-mile radius. You can view the radius in the map below. If your location is outside of this radius, a travel fee will be added to your reservation.
